Be sure to bring a current insurance card and co-pay to each visit, since our insurance plans require us to collect this information at the time of service. It is important to provide our office with accurate insurance and demographic information so that we may properly bill your insurance company.
If your appointment is due to a referral or you are a new patient to our practice, please bring copies of all pertinent medical records to your appointment for our physicians to review.
