New Patients

As a new patient, we understand that choosing an obstetrician or gynecologist can be difficult. We are glad that you have chosen us for your health care needs.

Please consider the following prior to your first visit with us:

• Please bring a current insurance card and a photo ID. Your insurance carrier requires that you present it to our front desk staff prior to your visit with the physician.
• You are responsible to make sure that any records, including referral forms for physician consults, laboratory results or ultrasound reports, are present in the office for your visit. You may want to bring copies with you to the office on the day of your appointment.
• Complete the new patient demographic form. Make sure all information is complete including all phone numbers and emergency contact numbers.

In addition, please arrive 15 minutes ahead of your scheduled appointment. This allows our front desk staff to enter all of your information into our automated system and reduces the time you will wait to see your physician.
