New Patients

For your first visit to our office, please be prepared to provide us the following information:

1. Completed patient information form. Please print clearly and use only blue or black ink. Do not mail forms back to the office. Please bring them with you at the time of your appointment.
2. Your current insurance card. If your insurance company has not issued your card we will need written verification, on insurance company letter-head, of coverage at your first visit.
3. A photo ID
4. Proper payment for any required co-payment, if you are a self-pay patient, or to handle any balance if you have insurance that we do not file to. Cash, check, credit and debit cards are accepted.
5. A parent or legal guardian must accompany individuals under the age of 18 for every visit.
