Pre-Op Instructions

In the event that your health care provider recommends surgery for your well-being, we are here to help you.

Prior to your surgery being scheduled, your insurance benefits will be verified. Upon receiving the information from your insurance company, a billing representative will notify you as to whether you have a deductible that still needs to be met and/or co-insurance that will be due before your surgery is scheduled. Our office policy is to collect your portion of the allowable charges before your procedure.
Please allow Missy, our surgery scheduler, two to three weeks to contact you in regards to scheduling your surgery. Your actual surgery date will be determined by your physician’s availability at your preferred hospital. If you anticipate having FMLA or short-term disability paperwork to be filled out, please obtain the paperwork from our front desk. This will enable us to provide your medical information to the appropriate person. Please return these forms to our office along with your required paperwork. Please note there is no charge for filling out the FMLA paperwork, but there is a $25 charge for any other form of paperwork.

Please make sure you have completely filled out your portion of all FMLA or Short-Term Disability paperwork. Failure to do so will result in a delay of completion. We will maintain a complete copy of all paperwork including appropriate signatures for our records.
